Agreement to Terms

By accessing or using the Skyway Animal Hospital LLC ("Skyway Animal Hospital," "we," "us," or "our") website (skywayah.com) or utilizing our veterinary services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our website or services.

Use of Our Website

Eligibility

You must be at least 18 years old to use our website and services independently, or have permission from a parent or legal guardian. By using our services, you represent that you are the legal owner of the pet(s) receiving care or are authorized to make medical decisions on behalf of the pet owner.

Acceptable Use

You agree not to:

  • Use the website for any unlawful purpose
  • Attempt to gain unauthorized access to our systems or data
  • Transmit any viruses, malware, or malicious code
  • Interfere with the proper functioning of the website
  • Collect information about other users without consent
  • Submit false or misleading information
  • Use automated systems to access the website without permission

Online Booking and Forms

Appointment requests and forms submitted through our website are subject to confirmation by our staff. Submission of an online form does not guarantee an appointment or service. We will contact you to confirm appointments and may request additional information as needed.

Veterinary Services

Consent to Treatment

By bringing your pet to Skyway Animal Hospital for care, you authorize our veterinarians and staff to examine, diagnose, and treat your pet. For procedures beyond routine examination, we will discuss treatment options and obtain your consent before proceeding. In emergency situations where you cannot be reached, you authorize us to provide necessary life-saving treatment.

Pet Owner Responsibilities

As a pet owner using our services, you agree to:

  • Provide accurate and complete information about your pet's medical history
  • Inform us of any known aggressive behavior or special handling requirements
  • Keep your pet current on rabies vaccination as required by Florida law
  • Maintain control of your pet while on our premises (leash for dogs, carrier for cats)
  • Arrive on time for scheduled appointments
  • Follow post-treatment care instructions provided by our staff
  • Notify us promptly of any changes in your pet's condition after treatment

Appointments

Scheduling

Appointments may be scheduled by phone, in person, or through our online booking system. Online appointment requests are not confirmed until you receive confirmation from our staff via phone, email, or text message. We reserve the right to reschedule appointments as necessary due to emergencies or unforeseen circumstances.

Cancellations and No-Shows

We request at least 24 hours' notice for appointment cancellations. This allows us to offer the time slot to other pets in need. Repeated no-shows or late cancellations may result in:

  • Requirement of a deposit for future appointments
  • Restriction to same-day scheduling only
  • In extreme cases, dismissal from the practice

Late Arrivals

If you arrive more than 15 minutes late for your appointment, we may need to reschedule to avoid delays for other patients. Please call us if you are running late so we can accommodate you if possible.

Fees and Payment

Payment Policy

Payment is due at the time of service unless other arrangements have been made in advance. We accept the following payment methods:

  • Cash
  • Credit cards (Visa, Mastercard, American Express, Discover)
  • Debit cards
  • CareCredit financing
  • Personal checks (with valid ID)

Estimates

For procedures and treatments, we provide written estimates upon request. Estimates are based on anticipated services and may change if your pet's condition requires additional diagnostics or treatment. We will contact you for approval before exceeding the original estimate by more than 10%.

Outstanding Balances

Accounts with outstanding balances may be subject to collection activities. We reserve the right to require payment of past-due balances before providing non-emergency services. Interest may be charged on accounts more than 30 days past due.

Emergency Services

Skyway Animal Hospital provides urgent care services during regular business hours. For after-hours emergencies, we recommend contacting an emergency veterinary hospital. We are not a 24-hour emergency facility.

Regular Hours: Monday-Friday 7:30 AM - 6:00 PM, Saturday 8:00 AM - 12:00 PM

For after-hours emergencies: Please contact VEG St. Petersburg at (727) 342-4334 or your nearest 24-hour emergency veterinary clinic.

Prescription Medications

Prescription medications require a valid veterinarian-client-patient relationship (VCPR) and a current examination within the past 12 months. We may dispense medications directly or provide prescriptions for use at outside pharmacies upon request, in accordance with Florida law.

Prescription refill requests require 24-48 hours' notice. Controlled substances have additional restrictions and may require an in-person visit.

SMS/Text Message Terms

By providing your mobile phone number and consenting to receive text messages from Skyway Animal Hospital, you agree to the following terms:

Program Description

The Skyway Animal Hospital SMS Program sends appointment-related and pet care messages including appointment reminders and confirmations, prescription ready notifications, follow-up care instructions, wellness and vaccination reminders, and occasional practice updates.

Message Frequency

Message frequency varies based on your appointment schedule and pet's care needs. You may receive up to 8 messages per month. Standard message and data rates may apply depending on your mobile carrier and plan.

Consent

By opting in to our SMS program, you expressly consent to receive recurring automated text messages from Skyway Animal Hospital at the mobile number provided. Consent is not a condition of purchase or service. You may opt in by providing your phone number on our appointment form, verbally consenting during a visit, or replying YES to our opt-in message.

Opt-Out Instructions

You may opt out at any time by replying OUT to any message. You will receive a one-time confirmation message and no further messages unless you opt in again. You may also opt out by calling us at (727) 327-5141 or emailing info@skywayah.com.

Carrier Disclaimer

Carriers (AT&T, T-Mobile, Verizon, etc.) are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission from your network operator and is not guaranteed.

Privacy

Medical Records

Your pet's medical records are maintained in accordance with Florida veterinary regulations. You may request copies of your pet's records or authorize transfer to another veterinary provider. Please see our Privacy Policy for details on how we handle your information.

Photography and Testimonials

We may occasionally take photographs or videos of pets in our care for educational, marketing, or social media purposes. If you do not wish for your pet to be photographed, please inform our staff. By providing a testimonial or review, you grant us permission to use your feedback in our marketing materials.

Intellectual Property

All content on this website, including text, graphics, logos, images, and software, is the property of Skyway Animal Hospital LLC and is protected by copyright and trademark laws. You may not reproduce, distribute, modify, or create derivative works without our prior written permission.

Third-Party Links

If a dispute arises between you and Skyway Animal Hospital, we encourage you to first contact us directly to seek resolution. If we cannot resolve the matter informally, you agree that any legal action shall be filed in the state or federal courts located in Pinellas County, Florida.

Your continued use of our website or services after changes constitutes acceptance of the modified Terms.

Severability
